Adult ADHD evaluation

A real evaluation, and an honest answer.

Most adults who suspect ADHD have never had a full workup. They have had a questionnaire, or twelve minutes, or an app. We do the whole thing, including the labs that almost nobody runs, because several treatable conditions look exactly like adult ADHD.

Read this before you book

How we work, in advance.

We would rather you know exactly how we work than have you find out after you have paid. If any of this is a dealbreaker, we are not the right practice for you, and that is a completely fine outcome.

  • There is homework, then one focused visit

    You complete the questionnaires at home so your appointment time goes to your actual history rather than paperwork. Then one 45-minute evaluation visit with your provider. You walk out with an answer.

  • What the labs are for

    There is no blood test for ADHD. The diagnosis comes from your history: what the symptoms are, how long they have been there, when they started, and where they cause problems. That is the whole basis for it.

    Labs do a different job. Thyroid disease, low iron, low B12, low vitamin D and untreated sleep apnea all produce inattention and fatigue that looks like ADHD. They are common, and they get missed. We order labs at your first visit so we can see whether something else is contributing, or explaining the whole picture.

    If the labs come back clean, the diagnosis still rests on your history. If something turns up, you know about it either way.

  • Bring what you have

    If you have been evaluated or treated before, bring what you have. None of it is required, and not having it does not delay your visit. A prior diagnosis is evidence we take seriously; if you would rather we request records directly, we will send you a release to sign.

  • Prescribing follows the evaluation

    Your provider decides based on what the evaluation and labs actually show, not on what was requested. If a diagnosis fits and treatment is clinically appropriate, options are part of the conversation, including approaches that are not medication. If treatment is indicated ongoing, you are seen at least every three months, because we do not manage anyone we are not actually seeing.

  • The evaluation may conclude that you do not have ADHD

    That happens, and when it does we tell you plainly and we tell you what we found instead. For a lot of people that turns out to be the more useful answer.

What the evaluation includes

One 45-minute evaluation. $200.

Before your evaluation you complete the screening questionnaires and history forms at home, which is what makes a focused 45-minute visit work. Your time with us is spent on judgment, not paperwork. All three parts below happen inside that one visit.

History and differential.

We go through the questionnaires you completed before we met, take your full history including childhood, because adult ADHD does not begin in adulthood, and build the differential. Labs are ordered and records are requested from any previous prescriber.

The structured interview.

The formal diagnostic interview, built around what your questionnaires flagged. Childhood onset in detail, how symptoms show up at work, at home, and in your relationships.

Findings and a plan.

What we found, what it means, and what we recommend, including approaches that are not medication. You leave with a written diagnostic summary you can take anywhere.

The differential

Things that look like adult ADHD.

This is the part other evaluations skip, and it is why people end up treated for years for something they never had. Every one of these is common, checkable, and treatable.

    Untreated obstructive sleep apnea

    In a healthy-looking adult it often does not look like snoring. It looks like brain fog, irritability, and being unable to hold attention through an afternoon. We screen everyone.

  • 02

    Low iron stores

    Ferritin can sit at the bottom of the range while a standard blood count reads perfectly normal. Restlessness, poor concentration, and exhaustion follow. Rarely checked, easily corrected.

    Thyroid dysfunction

    Both directions cause attention and energy problems. A single screening number often misses it, so we look wider.

    B12 and vitamin D deficiency

    Both are common, both affect cognition and mood, and both are simple to test and to fix.

    Anxiety, depression, and trauma

    These overlap heavily with ADHD and frequently travel with it. Sorting out which is driving what changes the entire treatment plan.

The labs are ordered at your first visit and reviewed at your third. Iron studies, thyroid, B12, folate, vitamin D, a metabolic panel, and screening for sleep apnea, mood, and substance use. We tell you what they cost before we order anything.

Sometimes we find one of these and the picture resolves. Sometimes we find one of these and ADHD, which is common. Sometimes the workup is clean and the diagnosis is straightforward. All three are useful outcomes, and you cannot know which one you are without doing the work.

The same $200 as every other follow-up visit at Align. If treatment is indicated, ongoing visits happen at least every three months. Labs are billed separately at cost and quoted before we order.

We are more expensive than an app that will send you a questionnaire, and considerably less expensive than full neuropsychological testing, which usually runs into the thousands and has a months-long wait. You see the price before you book and there is no bill afterward that you did not expect.

An honest note

What we will not do.

Prescribe without an evaluation. For anyone, for any reason. The 45-minute evaluation comes first.

Skip the labs. They are the reason this evaluation is worth more than a questionnaire, and they are not optional.

Continue a plan we have not reviewed. If you were treated elsewhere, we need the records, and we still do the evaluation.

Tell you what you want to hear. Some evaluations end with a diagnosis. Some end with sleep apnea, low ferritin, or an anxiety disorder nobody ever treated. We report what we find.

Common questions about the adult ADHD evaluation

  • How much does the evaluation cost? +
    The adult ADHD evaluation is $200, the same as every first visit at Align. It is one 45-minute visit. Labs are billed separately at cost and quoted before we order anything. If treatment is indicated, follow-up visits are $200 and run 30 minutes. There is no new-patient premium and no membership required.
  • What does the evaluation actually include? +
    A DSM-5-TR evaluation with validated instruments, a structured diagnostic interview, childhood history, and a medical differential, plus a written diagnostic summary for your records. All of it happens inside your 45-minute first visit, because the questionnaires are done ahead of time. You get one focused visit with a real answer at the end, not a rushed check-the-box appointment.
  • What is the homework you keep mentioning? +
    Before your evaluation you complete the screening questionnaires and history forms at home, on your own time. That is what makes a focused 45-minute visit work. Your time with your provider goes to clinical judgment rather than filling in forms. If you do not complete the packet, we will not schedule the visit.
  • Will I get a prescription? +
    Your provider decides based on what the evaluation and labs actually show. If a diagnosis fits and treatment is clinically appropriate, options are part of the conversation, including approaches that are not medication. If you are looking for a practice that will prescribe without an evaluation, we are not it, and we would rather say so now.
  • What if I was already diagnosed somewhere else? +
    Bring it. We request records from any previous evaluator or prescriber, and a prior diagnosis is real evidence we take seriously. What we will not do is continue a treatment plan we have never reviewed. The evaluation still happens.
  • What if the evaluation says I do not have ADHD? +
    Then we tell you plainly, and we tell you what we found instead. That happens, and for a lot of people it turns out to be the more useful answer. Finding untreated sleep apnea or a ferritin of 12 changes someone’s life more than a prescription would have.
  • How long does the whole thing take? +
    Your evaluation is usually scheduled within a week of your questionnaires coming back. Labs take about a week, and records from a previous provider can take a little longer. From questionnaires to a completed evaluation is typically two to three weeks.
  • If I am treated, how often will I be seen? +
    At least every three months, and more often at the start or after any change. That is our policy rather than a legal minimum, and it exists because we do not manage anyone we are not actually seeing. State law in both Utah and California also limits any single prescription for this class of medication to a 30-day supply, so the practical rhythm is a visit each quarter. Follow-up visits are $200 and run 30 minutes.
  • Do you take insurance? +
    No. Align is cash pay by design. It is what lets us charge the same $200 for every visit instead of a large new-patient fee. Some patients submit their own superbill for out-of-network consideration and we can provide the paperwork, but we make no promises about what any plan will do with it.
  • Who can be seen? +
    Adults 18 and over who are physically located in Utah or California at the time of the visit. California patients are seen by Rayna Fowler, FNP-C. We are not able to evaluate children or adolescents.
