Prescription requires medical evaluation. Not all patients qualify. Every plan is based on your history, evaluation, and lab results. Before you start any treatment, your provider will review the expected benefits, risks, and alternatives with you. Compounded medications are prescribed at the discretion of the licensed provider and are not FDA-approved as compounded preparations.

We’ll text you within one business day to confirm care path and pricing tier. Then we send your detailed intake form and, after your provider reviews it, a lab order. You draw labs, then we meet for your initial visit (paid at booking) where we cover your story, review your labs, and build your protocol in one go.

Common questions

How does this work if I’m never coming into a clinic? +

Everything is telehealth. Your initial visit is a video visit. Lab work is sent to LabCorp or Quest near you for a local draw. Prescriptions and protocols ship from licensed U.S. pharmacies. Your provider follows up by video every 30 to 90 days depending on your protocol.
Do you treat Lyme disease? +

Yes. Our chronic Lyme and coinfection protocols include Des Bio remedies, oral antimicrobials, herbals, and supportive care, all telehealth-deliverable. SOT injections require in-person administration; we’ll share timing once our American Fork clinic opens. We work with patients with confirmed Lyme, suspected Lyme, and post-treatment Lyme syndrome.
How much does this cost? +

One price. Every visit is $200, whether it is your first or your fifteenth. First visits run 45 minutes, follow-ups 30. No new-patient premium and no membership. Labs and any medications are billed separately at their own cost and quoted before we order. Programs that include medication are priced on their own service page.
Where are you licensed? +

Utah and California. Cory Fowler, FNP-C is licensed in Utah. Rayna Fowler, FNP-C is licensed in Utah and California. We treat patients physically located in either state via telehealth. California patients are seen by Rayna Fowler, FNP-C. License verification is available on request and via the Utah DOPL and California BRN.
When does the in-person clinic open? +

All visits are currently by telehealth for adults located in Utah and California. California patients are seen by Rayna Fowler, FNP-C. Our American Fork clinic is in development, and we’ll share an opening date once it’s confirmed. Sign up for our patient list and we’ll let you know when in-person consultations open.
Do you take insurance? +

We are cash-pay. Compounded medications, supplements, and specialty protocols are cash-pay. We do not contract with Medicare, Medicaid, or healthshare plans. We can provide a superbill on request that you may submit to your insurance for partial reimbursement of out-of-network visits.
